A highly suspicious thyroid
A 41-year-old woman presents with profound exhaustion.
For several months, everything seems to have gradually fallen out of balance: very restless sleep, near-insomnia, overwhelming anxiety, palpitations, chest pain, followed by the onset of new hypertension.
The course is fluctuating and puzzling: alternating episodes of intense anxious agitation and more depressive phases, with the patient describing herself as literally “at the end of her rope.” Symptomatic treatment with propranolol and hydroxyzine has been introduced, but the overall clinical picture remains only partially explained.
During the initial clinical examination, your colleague notes one finding that changes the interpretation of the case: a palpable goitre.
Between two consultations, she therefore asks you for a rapid thyroid ultrasound assessment.
A simple goitre discovered in the context of severe anxiety? An overlooked hyperthyroidism? Thyroiditis? Something else?
